Specific Information for International Students and Host Families

When you have been assigned a student, an information packet will be mailed to you.
Return the following forms from the packet:

CUSSD Authorization of Consent – signed by host parent or guardian (if student resides with the guardian), including doctor’s name and phone #
Directory Authorization/ID Card Information – signed by host parent or guardian (if student resides with the guardian)
Address/Information Check – data updated regularly from this form, required by Dept of Homeland Security
Affidavit of Relationship for Homestay Purposes - for students living with blood relatives

Forms to be signed by the parent or guardian - one copy to school, one copy to host family before student begins classes

Parent RenWeb instructions – you must register for this online to monitor student progress
Immunization Requirements
Health Clinic Information – useful in obtaining missing immunizations
Instructions for Athletic Eligibility ( CHS Athletic Packet , Request the CIF forms from Mrs. Ford)
18 year old Contract - all students who will turn 18 before graduation sign this
Change of address/phone/email - every time a student moves to your home, original to the school office
