A.  To receive the I-20, there are two steps:  (You must do Steps 1 and 2)

Step 1:  Apply:  (Initial acceptance eligibility is assessed after receiving the first 7 items)  

  1. Application fee:  $450 for grade 1-12 (add $20.00 if wiring money)
  2. Application for Admission Form 
  3. Copy of Passport Page (must be valid for at least 6 months prior to entry)
  4. Transcripts with translation (subjects, hours, and grades) in English!  See Transcript Guidelines
  5. Affidavit of Support for Sponsor
  6. Bank Statement of Sponsor in English (must match Affidavit of Support Sponsor)
  7. California School Immunization Record in English (must include dates)  See Immunization Requirements. Attention:  California requires 2nd Varicella if first immunization is given after the 13th birthday

Step 2a:  Commit:  (For the I-20 to be processed and mailed)

  1. Fill in and return the Commitment to Attend form
  2. Pay the international and text book fees: $3,600 (7-12); $1,100 (1-6).   (add $20.00 if wiring)   See Tuition and Fees
  3. Email or send the exact mailing address for I-20 in English to  Email Dr. Andrews   See   Contact Information  

Step 2b:  Finish Application Forms

  1. Student Commitment Form  
  2. Packet of Questionnaires:   
  3. Includes:
    (A) Student Questionnaire    
    (B) Parent Questionnaire
    (C) Teacher Questionnaire
    (D) Character Recommendation
     
  4. Go to the doctor to clear any missing Immunizations  
  5. Students in grades 9-12 participating in after-school athletics need to download the  Instructions for Athletic Eligibility 
  6.  

B.  Apply for Interview at Embassy for the Student Visa.  (Additional guidelines mailed with the I-20)

Notify the Primary Designated School Official (PDSO) when a decision is known Email Dr. Andrews 

C.  BEFORE BEGINNING CLASSES (after Visa is granted) the following must be complete:                  

  1. Pay tuition:     See Tuition and Fees
  2. Insurance purchased and forms submitted.                                         
  3. Designated Guardian in the United States OR Deposit of $1000.00 for return air fare and $1,000 liason fee.
  4. 18 Year-Old Contract (if 18 Years Old at any time before graduation)
  5. Housing - Student must be placed by  Homestay Company  of your choice; notify International Student Director, PDSO
  6. Affidavit of Relationship for Homestay (with supporting documents) if living with Blood Relative
  7. Homestay Release Form  (if living in a homestay) signed by parent or Designated Guardian
  8. Motor Vehicle Form   (if living in a homestay) signed by parent or Designated Guardian
  9. Fill out Address/Information Form for Immigration and web purposes - must be accurate and complete.

D.  Arrival at School Procedures and Class Registration: 

  1. Provide copy of Stamped I-20
  2. Show proof of insurance useable in the USA  OR  Purchase School International Insurance 
  3. Receive and complete Orientation packet (Mailed to homestay or guardian)
  4. Complete CUSSD's Parental Consent Form 
  5. Take the English and Math Placement Tests before school starts (Report dates in Orientation packet)
  6. Purchase school uniforms

 

IMPORTANTNever enter the United States on a tourist visa if you intend to attend school.
If you enter on a B-1/B-2 visa, make sure it says “Prospective Student” on the visa page.
